Considering her opinion that she needs a boob job, she might actually be self-conscious about her naked body. That's just my best guess. Granted looking at her I couldn't very well pick any area that is needing improvement at all tbh, but then again how ppl see us and how we see ourselves are generally not similar. I myself even had two girlfriends in my life that preferred to keep a bra or shirt on even during intimacy. One was a red head with a ton of freckles 32c cups with freckles all over her boobs. I thought she was sexy as hell, but she didn't just think they were hideous; She was completely convinced of it regardless of my compliments. Shrug.
