it was squishy123, says I got banned for "spam", which confuses me cause i like never post anything

After looking it up, it could also be that you had three accounts here. squishy123, squishy1234 and another.
Is there a formatting template I can use for the appeal ticket?
If not, what should I include? What should the title be, etc.
